This page contains all necessary forms for the Belton MS Band. Items marked with an asterisk (*) are required to be read and filled out when a student registers for Band.

*Belton ISD Extracurricular Code of Conduct – the reading and signing of this document is required by all students and parents involved with extracurricular programs in Belton ISD.

*Belton ISD Extracurricular Code of Conduct Acceptance Form – this required form is to be filled out by both student and parent acknowledging their acceptance of this Code of Conduct

*Belton Middle School Band Handbook 2025-2026 – reading of this document is required by both students and parents, and contains all guidelines and policies that dictate procedure for the Belton MS Band.

*Band Handbook Acceptance Form – this required form is to be filled out by both student and parent at the same time indicating their agreement to all of our guidelines and policies.

*School-Owned Instrument Usage Agreement Form – this form is required for both parents and students who play an instrument that is owned by Belton ISD. It states that school equipment will be the responsibility of the student and parent while checked out to them. Students will be checked out an instrument after this form has been completed.

*School-Owned Instrument Payment Plan Form – this form is also required for parents of students who play an instrument checked out from BMS. Parents can select their preferred payment plan of $100 for the school year that is non-refundable. Students will be checked out an instrument after this form has been completed.

*Private Lesson Form – this required form is for parents to consider private lessons for their student and should be filled out, even if the student/parent is not interested in taking lessons. Most lessons can be taught during the school day!

  • Private Lesson Financial Assistance Application – fill out this form if you are in need of financial assistance to help pay for lessons. Please note that not all application will be approved and this is done on a first-come, first-serve basis as funds are limited. Please allow up to 2 weeks for your application to be processed and reviewed by the committee. All submissions are confidential.

*Uniform Shirt Order Form – this online form is required for students and parents to order their uniform shirt, including Beginner T-Shirt ($15), Symphonic/Wind Ensemble Polo Shirt ($25), and Jazz Band Shirt ($30). Resubmit for additional shirt orders.

*Auto Decal Order Form – this optional form is for parents who wish to order the regular Band logo or Jazz Band logo customizable with their student’s name and instrument and costs $15 each. Resubmit for additional decal orders.

*Belton ISD Volunteer Registration – this form is for parents/guardians to register with Belton ISD as a district chaperone to run the background check that is required to be done every school year. Current district employees do not need to fill out this form.

Permission to Leave Band Event with Parent – this form is to be filled out when parents wish to pick up their student directly from a band event rather than ride the bus back.

After Performance Evaluation Form – this form is for students to self-assess their recent band performance.

Music Listening Questions Form – this form is for students to answer questions about the music they hear in class.

Rehearsal Guidelines Self-Assessment Form – this form is for students to evaluate themselves on their behavior efforts in following our rehearsal etiquette guidelines.

Music Request Form – this form is for students who have lost or misplaced their music and need another copy… or maybe someone stole it!
