This page contains all necessary forms for the Belton MS Band. Items marked with an asterisk (*) are required to be read and filled out when a student registers for Band.
*Belton Middle School Band Handbook 2021-2022 – reading of this document is required by both students and parents, and contains all guidelines and policies that dictate procedure for the Belton MS Band.
*Band Handbook Acceptance Form – this required online form is to be filled out by both student and parent at the same time indicating their agreement to all of our guidelines and policies.
*School-Owned Instrument Payment Plan Form – this online form is required for parents of students who play an instrument owned by BMS. Parents can select their preferred payment plan of $100 for the school year that is non-refundable. Students will not be allowed to check out an instrument until this form is completed by the parent.
*Private Lesson Form – this required online form is for parents to consider private lessons for their student.
*Uniform Shirt Order Form – this online form is required for students and parents to order their uniform shirt, including Beginner T-Shirt, Symphonic/Wind Ensemble Polo Shirt, and Jazz Band Shirt.
*Auto Decal Order Form – this online form is for parents who wish to order the regular Band logo or Jazz Band logo customizable with their student’s name and instrument.
*Parent Chaperone Sign-Up Form – this form is for parents to sign-up as a band chaperone to help with our various events throughout the school year. We can’t do it without your help!
*RevTrack Online Store – this is the online web store through Belton ISD for parents who wish to pay for their band fees online using a credit/debit card, including the School-Owned Instrument Usage Fee, Uniform Shirts, and the optional Auto Decal. Please Note: Sales tax will be added, as well as a 4% service fee to your purchase when you use this option. Alternatively, parents can choose to pay with cash or a check made out to BMS with a valid phone number by putting their payment in a sealed envelope and writing their student’s name and the item(s) they are purchasing on the front side of the envelope.
Permission to Leave Band Event with Parent – this form is to be filled out when parents wish to pick up their student directly from a band event rather than ride the bus back.
After Performance Evaluation Form – this online form is for students to self-assess their recent band performance.
Music Listening Questions Form – this online form is for students to answer questions about the music they hear in class.
Rehearsal Guidelines Self-Assessment Form – this online form is for students to evaluate themselves on their efforts in following our rehearsal etiquette guidelines.
Music Request Form – this online form is for students who have lost or misplaced their music and need another copy… or maybe someone stole it!
